Reading this website, submitting a form, or booking an appointment online does not create a provider–patient relationship. That relationship begins when you're evaluated and treated at our clinic.

Appointment times selected through our online calendar are requests until our team confirms them — usually by text, email, or a quick phone call. We honor requested times whenever we can; if a slot needs to change, we'll contact you promptly to find one that works for you.

The content on this website — text, graphics, page designs, and the Elevate Therapy Doral name and logo — is owned by us or our licensors. You're welcome to view, download, and print pages for your own personal, non-commercial use. Please don't republish, copy, or use our content for commercial purposes without our written permission.
