This Privacy Policy explains how Elevate Therapy Doral ("we," "us," or "our") handles information collected through this website. It applies to the website only. The care you receive inside the clinic — evaluations, treatment notes, billing, insurance — is documented in our medical record systems, protected under HIPAA, and covered by a separate document, our Notice of Privacy Practices, available at the clinic. There's more on that difference in section 06.

This website itself does not collect personal information through on-site forms. When you're ready to book, we send you to our patient portal — a separate, secure scheduling service. Any details you enter there (such as your name, contact information, and the appointment time you choose) are collected and handled by that portal under its own privacy practices, not by this website. We never ask for payment information, government IDs, or passwords anywhere on this site.

If you reach out using the phone or email links on this site, you're contacting us directly — those details aren't processed through any third-party form platform.

Like nearly every website, this one relies on a few outside services to load and run. When your browser requests those services, they automatically receive standard technical information — things like your IP address, browser type, device type, and the pages you visit. We don't use this information to identify you personally. Section 07 lists the services involved.

We use the information you give us to do exactly what you'd expect:

• → Respond to your questions and messages
• → Schedule, confirm, and reschedule appointments
• → Send appointment confirmations and reminders by text message and email
• → Keep a record of your request so we're prepared when you call or come in

We don't use the information you share for anything unrelated to your care and your communication with our clinic.

When you give us your phone number — by booking through our patient portal or by contacting us directly — you may receive appointment-related text messages from us: confirmations, reminders, and scheduling questions. Message and data rates from your carrier may apply.

You can opt out at any time by replying STOP to any message; you may receive one final text confirming you've been unsubscribed. Opting out never affects your ability to get care — we'll simply reach you by phone or email instead.

We do not sell, rent, or trade your personal information — not to advertisers, not to data brokers, not to anyone. The only outside parties involved are the technical service providers described below, and we may disclose information if the law requires it.

A few third-party services help this site work. When your browser loads them, they may technically receive your IP address and basic browser information:

• → Our patient portal — when you click a booking link, you're taken to our external scheduling portal, which collects and handles your booking details under its own privacy policy.
• → Google Maps — the map in our Visit section loads from Google, which may receive your IP address and basic browser information.
• → Font and styling libraries — third-party services that deliver the typefaces and styling this site uses.

Each of these providers publishes its own privacy policy that governs how it handles that technical data.

Our patient portal and email are for scheduling and general questions only — they are not a channel for medical information. Please don't include symptoms, diagnoses, medications, or other health details when you book or write to us. If you need to discuss something medical, call us at (786) 828-6161 and we'll talk it through properly.

Your health records are created and kept in the clinic's medical record systems, which are handled under HIPAA. Our Notice of Privacy Practices — the document that explains your rights over those records — is available at the clinic; just ask at the front desk.

We keep any information you share with us directly (such as a phone or email message) for as long as we need it to respond to you and manage scheduling, and as any applicable law requires. If you'd like information you've sent us deleted, email [email protected] or call us, and we'll take care of it wherever the law allows. (Clinical and medical records follow their own legally required retention rules and can't always be deleted on request.)

This website is not directed to children, and we don't knowingly collect personal information from children under 13 online. We're glad to treat younger patients at the clinic — a parent or guardian should submit any booking or contact form on a minor's behalf.

If we change this policy, we'll post the updated version on this page with a new effective date. Meaningful changes will be reflected here before they take effect.
