Will I see a licensed therapist every visit? +
Yes — every visit is with a licensed therapist, never an aide. Your plan is created and led by one of our doctors (DPT or OTD) and delivered hands-on by our therapy team. It's the single biggest reason patients tell us Elevate "feels different."
Do you take my insurance? +
We're in-network with most major plans — Aetna, Blue Cross Blue Shield of Florida, Cigna, UnitedHealthcare, Humana, Medicare and Medicaid of Florida, OSCAR, Solis, Sun Health, and Curative. We also work with all workers' comp networks (MedRisk, CorVel, One Call, Streamline, Total Network, and equivalents) and take auto accident and LOP cases directly through your attorney. Some premium services — like Shockwave and the Recovery Membership — are cash-pay. We verify your benefits before your first visit so there are no surprises.
Do I need a referral to book? +
No referral needed. Florida is a direct-access state — you can book with us straight away. If you'd like to use insurance, we'll quickly verify whether your specific plan requires a referral for coverage. For cash-pay services (Initial Evaluation, Shockwave, Recovery Membership), you can book today and walk in this week.

How fast can I get an appointment? +
Same-week intake is the standard at Elevate. No black-hole front desks, no weeks of back-and-forth. Most initial evaluations happen within 2–4 days of booking.
What's different about your shockwave therapy? +
We use focused shockwave for chronic tendon pain — plantar fasciitis, tennis elbow, patellar tendinitis, Achilles, and shoulder tendinopathy. It pairs with manual therapy and a strength program, never delivered as a one-and-done gimmick.
I've tried PT before and it didn't help. Why would this be different? +
